Exciting Leadership opportunity for an experienced Care Manager to join our Residential Care Team at Bellevue Court, located in Gawler.

About the role

As a Care Manager with Southern Cross Care, you will work with the Residential Services Manager to implement a healthy and active ageing service delivery model, which ensures resident choice, independence, and quality of life are maintained at the highest level.

You will work with your colleagues and residents to develop and implement goal-oriented care plans. You will lead, support and mentor the care team to ensure optimal resident outcomes, and will lead the early intervention working group to develop recovery and fitness pathways for residents.

About you

You should have at least 3 to 5 years experience as Care Manager within Aged Care and sound understanding of Aged Care legislation and the new SIRS.

You will have experience with the Aged Care Funding Instrument (ACFI) and will be passionate about providing high-quality person-centred care. You will have effective leadership skills and aged care experience including the ability to work at an advanced level of nursing practice in the care and management of older people. You will also have exceptional organisational and time management skills and the ability to manage a range of priorities.

With a Bachelor of Nursing or equivalent, current AHPRA registration and an impressive reputation for person-centred service in aged care, you will also demonstrate high-level listening, communication, team building and time management skills.

Previous experience in Memory Support Unit (MSU) would be an advantage.
