Case Managers work collaboratively with key stakeholders to achieve positive outcomes for all clients and stakeholders.

Position Description:

The Case Manager will utilise their extensive work history in case management settings, working with clients with mental health conditions, vulnerabilities and/or complex cases.

You will be experienced in considering the social determinants of health and wellbeing.

Responsibilities:

  • Provide an escalation point for clients at serious risk of harm to self and others
  • Manage a limited case load of high complex clients
  • Contribute to the preparation of high level client summaries and briefs, including maintaining high quality documentation
  • Undertake a range of client assessments including intake assessments and risk assessments.
  • Provide specialised case management and or clinical expertise

Mandatory:

  • Tertiary qualification (Bachelor’s degree or higher) in a relevant health field including Nursing, Psychology, OT or Social Work
  • At least 3 years’ experience working as a senior case manager or equivalent clinical role
  • Extensive experience working within a mental health or community case management setting
  • Current Police Check, or ability to obtain one
  • Experience in coordinating and assisting in the management of a caseload of complex cases
  • Excellent written and oral communication skills, including capacity to liaise with and develop productive relationships with other organisations
  • Proficiency in the use of IT systems including Microsoft Office

Highly Desirable:

Registration with the Australian Health Practitioners Registration Agency (AHPRA), or eligibility for registration with the Australian Association of Social Workers (AASW).

  • Experience working with clients with challenging behavioural issues
  • Experience with and understanding of the principles of Trauma Informed Care
  • Experience in provision of reflective practice supervision

Expertise in supporting clients experiencing the impacts of substance abuse, mood and anxiety disorders (including PTSD).
